Auto-Renew Membership Change Request Form

Hi there! To cancel or freeze your auto-renew membership, please submit the form below. As a reminder, membership cancellations must be requested is at least 7 days prior to your next billing date, and freezes must be requested at least 48 hours prior. Otherwise, the upcoming billing cycle will be processed as scheduled and the cancellation will go into effect the following month. No exceptions can be made.
